Service Request Form |
- If this is the first time you are requesting disability
accommodations:
- Submit the Service Request Form (copies available
on-line, at the Disability
Coordinator’s office (Room 208) or at the Student
Services office. You will be scheduled for an intake
appointment once the Disability Coordinator receives the
Service Request Form.
- If this is not your first time requesting disability
accommodations:
- Meet with the Disabilities Coordinator in Room 208 before registering each semester.

Listed below are some physical and academic accommodations
provided to students in the past:
- Early advisement
- Early registration
- Use of tape recorder in the classroom
- Use of textbooks on tape
- Time extensions for:
- Completing exams
- Completing assignments--Early notice preferred
method to meet regular deadlines
- Library material
- Completing program requirements
- Completing course requirements
- Use of an in-class note taker (a peer may share notes)
- Use of a calculator (for in-class assignments and exams)
- Evaluation of in-class assignments and exams should
exclude spelling
- Alternative testing methods and/or setting:
- Testing in a quiet place
- Oral questions and dictated answered to supplement
or replace written tests
